Waiver

Please read the following before signing:

The user warrants that he or she is in good physical condition and has obtained the approval of his
or her attending physician to engage in physical activities as offered by Reps Fitness and
Conditioning and/or CrossFit 709.

I agree, that while using the facility or the services provided thereby, to conduct myself in a manner
that does not present a danger or create a nuisance to the facility, its employees and other persons
using the facility or Reps Fitness and Conditioning and/or CrossFit 709 members.
I acknowledge

– That Reps Fitness and Conditioning and/or CrossFit 709 is not liable for any theft or loss of
personal items which may occur on or about the facility’s premises

– That no claim has been made by Reps Fitness and Conditioning and/or CrossFit 709, its
directors, officers, employees, or agents to offer assessment or treatment of any mental or physical
disease or condition.

– That I am using the facility at my own risk and thereby releasing Reps Fitness and
Conditioning and/or CrossFit 709, its directors, officers, employees or agents of and from any
claims arising from my usage of this facility.
